Advocacy

Independent Advocacy Perth & Kinross Independent advocacy involves speaking about issues that concern you and being able to speak up for yourself by being given information and support. Also, advocacy can involve someone speaking on your behalf to protect your rights and interests when you are unable, for whatever reason, to do this for yourself. Independent advocates can help with all sorts of issues and situations, for example, health, social work, housing, education, and family issues. Arts Therapy

Art therapy encourages individuals to express difficult emotions, promoting physical, mental, social and emotional wellbeing through the creative use of art materials, music, role-play or story telling. Arts Therapies in the UK is a State Registered Health Profession and a member of the Health Professions Council. Art Therapy Art therapy provides a secure environment – usually a private room or study studio – and a variety of basic art materials, such as clay, paint and paper, collage, and themselves. Music Therapy Music therapy is the use of sounds and music to support and encourage physical, mental, social and emotional wellbeing. The music therapist works with a variety of clients in groups or individually. Music is used in a clinical setting to stimulate and engage clients and to help them towards realising their potential. Drama Therapy Drama therapy makes use of role-play, voice work, myth, ritual and story telling. Drama therapists work in a number of areas including mental health, training centres, prisons, childcare, learning disabilities and private practice. They use the healing aspects of drama and theatre as a therapeutic process that fosters creativity, imagination learning, insight and growth. DART/Meadowell

DART/Meadowell offers Day Services for people with severe and/or enduring mental health problems living in rural Perth & Kinross aged between 16-65 years. The service aims to enhance individuals' lives by providing opportunities for social and recreational activities and offers basic living skills groups as well as stress management, anger management, women's groups and therapeutic support. All referrals are received by the Team Leader and discussed at the weekly team meeting where, if accepted, are allocated to a specific worker. Fourways

Fourways Day Service offers a service to adults with severe and/or enduring mental health difficulties aged between 16-65 years. Based in Aberfeldy, it provides a day service to North West Perthshire including, Dunkeld, Pitlochry, Kinloch Rannoch and the surrounding areas. The support and activities provided are social, recreational and therapeutically focussed. For example, there are social groups, fishing group, leisure group, and gym and computer access. Also, provided is individual support to engage in therapeutically focussed work and groups such as an anxiety management group. Referrals are made directly to the Day Service. Rannoch Road

Rannoch Road Resource Centre offers a day service for Older Adults, over 65 years of age. The main aim of the centre is to provide support to enable individuals to live as independently as possible within their own homes. This is achieved in a number of ways including, short term rehabilitation following a hospital stay or illness, carer relief, support through acute mental illness e.g. dementia or depression and also, support following bereavement. The service is open to vulnerable older people where there are concerns of risk to safety or self-neglect. Transport to the centre is usually provided in the form of a minibus taxi, volunteer driver or adapted car. Direct referrals can be made from Healthcare facilities under the Single Shared Assessment scheme. Referrals can also be made through the Adult Care duty team at Pullar House. Drug and Alcohol Team

The Drug and Alcohol Team offer confidential support and counselling to anyone aged 16-65 years with a drug and/or alcohol problem or to anyone living with, working with or caring for someone with drug or alcohol difficulties. The service operates Mon – Fri 9.00am – 5.00pm. Open referrals are welcome by telephone, letter or in person during duty system times and a duty worker is available Mon – Thurs 9.00am – 1.00pm with answer machine service outwith these times. Tayside Alcohol Problems Service (T.A.P.S.)

Tayside Alcohol Problems Service provides a confidential counselling and support service for anyone within the Tayside region who requires advice or help with an alcohol problem. The T.A.P.S. team consists of medical and nursing staff working in both community and residential settings, offering individual counselling and support groups along with support and advice for relatives. T.A.P.S. operates a direct referral system which means you can contact the service yourself or through other agencies such as your G.P. or Social Work Department. Tayside Drug Problem Service (TDPS)

Tayside Drug Problem Service provides support and advice to individuals with opiate dependence problems in the community. A multi-disciplinary service, TDPS offers emotional, psychological and practical support to those affected by a drug problem. Central Healthcare

Central Healthcare is a nurse-led service that aims to meet the healthcare needs of vulnerable individuals and families within the Perth & Kinross area. The service includes outreach, drop-in, G.P. and nurse led clinics. Individuals must be homeless, temporarily accommodated or at risk of being made homeless. Community Liaison Team (CLT) The Community Liaison Team are responsible for assessing patients referred from within PRI and Ninewells Hospitals to facilitate and support discharge at the appropriate time, to the most appropriate environment. The purpose of the team is to: Assess the patients as appropriate for each individual service. Support and educate all staff involved with the CLT in relation to CLT responsibilities. Support the Acute Ward staff in understanding the service criteria. Early identification of delayed discharge patients. The team is a multi disciplinary team of Nurses, Occupational Therapists and Care Management that co-ordinates care for patients being discharged or transferred on to the following services: Step-down to Community Hospitals Step-down to Medicine for the Elderly Intensive Care at Home Service for Perth City (ICAHS) Assessment of care needs for care packages or long term care Referrals are taken from Doctors, Nurses, Occupational Therapists, Physiotherapists from the Acute and Medicine for the Elderly Wards in Perth Royal Infirmary and the Acute Wards for Ninewells. Community Liaison Team Care Management

Care Management within the Community Liaison Team offers assessments of need to patients/clients over sixteen years of age in Perth Royal Infirmary or Murray Royal Hospital who may require Community Care services to enable discharge from hospital. The majority of referrals are made by ward staff. However it is also possible for clients or their relatives to make referrals. Information is gathered from a number of sources including the client, their family, medical and nursing staff, Occupational Therapists, Physiotherapists and professionals in the community such as Home Care Organisers, Care Managers, District Nurses, Community Psychiatric Nurses and GPs. From this information needs are identified and a care package implemented that aims to meet these needs after discharge from hospital. The care package can comprise of services to try and help maintain someone at home or, if their needs are too great to be met at home, a placement in a Residential Home or Nursing Home can be arranged. Older People Mental Health Teams

The aim of the teams is to enhance the quality of life for older persons with mental health difficulties, and their carers. There are four Older Peoples Mental Health Teams based at: Murray Royal Hospital, Perth. Contact: Val Aitken on 01738 562201 Blairgowrie Community Hospital. Contact Ann Robinson on 01350 728095 These teams provide assessment, treatment and monitoring as part of an assessment agreed by the older person. All the teams have close links with Day Services and Inpatient Services across P&K All teams work between 8:30am until 4:40pm, Monday to Friday. There are answering machines available out with these hours where a message can be left. Patients must be either over 65 years with mental health problems, or under 65 years and have memory problems. Referrals can be made through your GP, or any other health/social work professional. If you wish, you can self refer. All referrals made are discussed at a weekly team meeting, and your GP is informed of the outcome of this.

Perth City Locality Day Services

This service is a local day facility based at Murray Royal Hospital which enables people with mental health support needs to access opportunities and other services in the community. It aims to help people regain confidence and skills and promotes social inclusion in the community. A generalised aim is to encourage people to realise their full potential. Some examples of activities that the service helps individuals link into are : voluntary work, work preparation, leisure, recreation, education and social activities.The team consists of support worker Liz Swann who has extensive mental health experience and Development Officer, Pam Lamond who is a registered Mental Health Nurse.

Those who have used this service rate it highly, praising the individualised approach based on the principles of recovery and directing/ assisting the service user towards full citizenship. At present the srervice is only available to residents of Perth City or people registered with a Perth based GP but people in other areas of Perth & Kinross may be considered dependant on resource. Psychological Services

The purpose of Clinical Psychology is to reduce psychological distress and to enhance and promote psychological wellbeing by the systematic application of knowledge derived from psychological theory and data. A wide range of psychological difficulties may be dealt with through the skills and expertise of psychologists in several different specialisms including anxiety, depression, relationship problems, learning disabilities, child and family problems, and serious mental illness. Specialisms in Tayside Psychological Services include the Child and Adolescent Mental Health Service, Adult Psychological Therapy Service, Learning Disabilities, Health Psychology, Neuropsychology, Older Adult Service, Forensic Services and involvement in Community Mental Health Services. Perth & Kinross Leisure serves the whole of Perthshire and beyond – helping you to liveACTIVE lives! Operating in partnership, the company provide services on behalf of Perth & Kinross Council, with an annual fee paid for these services. The Youth, Community, Arts and Culture section of Education and Children's Services provide the client monitoring function for the Council.
